Significance of Screening Tests in Sudden Sensorineural Hearing Loss

Abstract
Objective: Sensorineural hearing loss, whether sudden or progressive in onset, is known to be caused by numerous factors. For patients with sudden sensorineural hearing loss (SSNHL), current practice guidelines are to perform through blood and serologic tests to exclude treatable causes or underlying disorders. But, the effectiveness of these screening tests has been doubted until nowadays. This study aims to know the necessity of the systemic blood and serologic screening tests and the relationship between the results of screening tests and the prognosis.

Materials and Methods: We reviewed retrospectively 440 patients who were diagnosed as SSNHL. We reviewed their charts for past histories, serologic tests, lipid panels, and thyroid function tests. We analyzed the positive results of screening tests comparing between group I (hearing improved more than 10 dB HL) and group II (not improved or improved less than 10 dB HL). Pearson Chi- Square test was used for statistics.

Results: There were no significant differences between two groups in past history, VDRL test, viral marker tests, and lipid profiles tests. Abnormal TSH level in group II were statistically significant (p=0.001).

Conclusion: On the basis of our results and the cost of systematic screening, we propose that serologic tests, lipid profiles and thyroid function test can be limited to the patients with suspected histories or symptoms.
